On or by the first day of school, the nurse’s office at the school your child attends must have the following on file:
- doctor’s orders
- test kit and batteries, supplies
- protein/carbohydrate snacks
- any other supplies needed (glucose tabs, ketostix, extra test strips, lancets, syringes, Insulin and Glucagon as prescribed, and/or Insulin pump supplies).
