7th grade physicals and sports physical appointments available

The Robert J. Kaiser Middle School will have physician Dr. Gill at the school on two dates for physicals: 

Students who have not yet submitted their 7th grade state-required physicals may stop by the nurse’s office to be added to the list on this date. Parents can call 845-796-3058, ext. 5 to request a slot. 

Beginning in February, students in need of a sports physical may request a form from their Physical Education class to bring home. The completed form, signed by a parent or guardian, must be returned to the nurses office in order to receive an appointment slot for a sports physical.
